Referring a Patient 

References can be made directly to Essendon Private Clinic if admission is required for a patient. We have created a referral letter that can be used when admitting a patient to our facilities. The letter can be downloaded here, and can save you time ensuring that your patient can be referred to a specialist who is best suited to their needs. 

Alternatively, you may simply write up your own letter containing the following information:

  • Patient’s name
  • Address
  • Contact details 
  • Health insurance details
  • Date of birth
  • Outline of current issues
  • Current regime
  • Previous treatments
  • Any risk history or potential risk factors

Once we have received the referral from you, we will then facilitate the admission and engage a Psychiatrist to care for your patient if they do not already have their own.

Following a patient discharge, a medical discharge summary will be sent to the referrer.

Specialist Accreditation

Essendon Private Clinic encourages Psychiatrists to apply for accreditation to our facility. Please download and complete the following forms:
